Why a framework

A practice framework turns values into habits.

Anyone can put "trauma-informed" in a brochure. Our five-phase framework is how we make sure the words actually mean something on the ground — in the welcome call, in the assessment, in the Wednesday-afternoon check-in. The phases are sequential at first, but Phases 4 and 5 run concurrently for as long as you're with us.

Phase One

Engagement & Relationship Building

A warm welcome, a clear explanation of what to expect, and a safety-and-consent conversation at your pace. From the first 24 hours we invite you to share what matters most to you, and we acknowledge every referral promptly so no one is left wondering "what happens now?"

Phase Two

Comprehensive Assessment

A holistic review across emotional, physical, social and functional domains. We use clinically validated tools (LEC-5, PHQ-9, GAD-7, WHODAS 2.0) alongside a Strengths & Needs Matrix and collaborative risk-and-safety planning. We share the findings with you — and nothing goes into a plan without your agreement.

Phase Three

Co-Designed Support Planning

A 60 to 90 minute planning meeting where we shape the plan together. SMART goals you actually recognise as yours, modalities you've chosen, and clear roles and responsibilities for everyone involved. The plan is signed off digitally, stored securely, and can be revisited whenever you want.

Phase Four

Evidence-Based Delivery

Safety and stabilisation first — grounding, breathing, crisis de-escalation — then skill-building, connection, and condition-specific supports tailored to each goal. Delivered in-home, via telehealth, or in community venues. Always at your pace, always with your permission, always something you can opt out of without penalty.

Phase Five

Monitoring, Review & Continuous Improvement

Weekly check-ins, fortnightly reviews against your goals, and quarterly outcomes audits that feed back into how we practise. Our Clinical Lead leads monthly reflective supervision with every case manager — so the learning from your journey improves the support the next person receives.
